COVID-19 Vaccination Clinic 4/13/2021 at Clifton Park Senior Community Center

The Town of Clifton Park will be hosting a Covid Vaccination Clinic Tuesday, April 13th in cooperation with Caring Wellness Pharmacy.  The 300 available doses of the Moderna vaccine, will be provided by appointment only.  To select an appointment please select a time from the chart below. 

 Vaccine appointments are ONLY available for Individuals Aged 18+ or Individuals with Pre-existing Conditions (Letter from your Doctor) or Comorbidities (Letter from your Doctor).  

An appointment confirmation email will be sent after you schedule your appointment. To shorten your wait time, please be sure to fill out the necessary paperwork prior to your appointment: confirmation of New York State’s COVID-19 Vaccination form and the COVID-19 vaccine consent form. Please bring a government-issued photo ID to your appointment, your Red-White and Blue Medicare card, and/or any other insurance card to your appointment (Including the insurance card you take to your doctor’s office and pharmacy). For those individuals who are unable to complete the paperwork online, there will be forms available at the clinic.

The vaccinations will be performed at the Clifton Park Senior Community Center on the Clifton Common.  The address is 6 Clifton Common Court.  Please arrive five minutes prior to the time of your appointment.  Town staff will greet participants as you enter the facility. 

The Town of Clifton Park has assisted more than 1000 people make vaccination appointments.  Any Clifton Park residents who are having difficulty establishing an appointment can contact Supervisor Barrett’s office at 518-371-6651.    

To check availability of appointments for the 1st Moderna vaccine on Tuesday April 13, 2021, please select a time below. Once a time has reached its capacity limit, a message will be pop up indicating that you are not authorized to access that form. Please try another time. 

HIT REFRESH on your browser if you don't see many openings.  We have 12 patients allowed for each 15-minute time slot. When 12 appointments are booked, it will not allow any more appointments for that time.  As of Saturday morning  there are about 100 appointments available.

 New York State Department of Health (NYSDOH) and Saratoga County Public Health Services continue to actively work on the COVID-19 B.1.1.7 variant investigation involving N. Fox Jewelers in Saratoga Springs.

COVID-19 Information for Saratoga County - January 4, 2021

Earlier today, Governor Cuomo announced that a variant of the COVID-19 virus first discovered in the UK has been identified in Saratoga Springs. Saratoga County Public Health Services is actively working with New York State Department of Health (NYSDOH) on the investigation.

Employees of N. Fox Jewelers are being swabbed today and NYSDOH will be offering free testing to patrons who were at the store between December 18 and December 24.
